Terms and Conditions: The following Terms and Conditions of use apply to this Reduced Rate Dental Plan and the Web Site operated by New York Dental Network, LLC, hereinafter referred to as "NYDN." Please read the following terms and conditions of use. Please contact NYDN directly if there is any question about the contents of this Web Site. User's Agreement: NYDN is a reduced-rate Dental Network; it is NOT an insurance plan and is not regulated by any State Insurance Department or similar insurance regulatory body. The principal function of NYDN is to make available Dental Services at a reduced rate, per terms published on NYDN website. NYDN Web Site users represent that they have read and understood, and agree to be legally bound, and are capable of being legally bound, by the terms and conditions posted here. NYDN Membership Payment Agreement: Unless notified otherwise in writing by NYDN, all NYDN membership payments are made by NYDN members directly at NYDN Web Site by use of specified Credit Card, Debit Card, or eCheck. All NYDN Membership fee payments are preauthorized and recurring, until further notification by NYDN member. No agent, individual or other entity is authorized to collect or process NYDN membership fees. Member may request cancellation and refund (minus a $10. service charge) during first 45 days of membership, and NYDN will comply. Web Site Content: NYDN may revise or make changes to any part of this Web Site, without notice and without liability to users of NYDN Web Site. Suitability of Content: NYDN does not practice dentistry or offer medical or dental advice of any kind.
