| Eligibility |
Group: Eligible Groups include Corporations, Partnerships, LLC's, Sole-Proprietorships, Union and Trade Organizations, Government Organizations, Religious, Charitable, and Fraternal Organizations, and Educational Institutions. Each group member will enroll (at reduced rate) and make payment arrangements at our website. If member leaves a group, he/she can continue membership at the reduced rate. We will notify Groups periodically about member enrollment.
Regular: Membership is open to Individuals and Families. All members of a household, related or not, are eligible to enroll at the family-household rate. College students, living at home or away from home, are included when family-household coverage is elected.
Senior: All Seniors 62+ enroll at a reduced rate. The reduced rate applies to all members of a household in which at least one Senior is enrolled.

Membership Fee Payments are pre-authorized upon enrollment via our secure website.

We will draft membership fees from your Credit Card or Debit Card until further notice from you. NYDN communicates with members via e-mail or phone. NYDN does not send conventional bills.

60-Day Trial Membership Period: Our goal is to provide a high quality, user friendly, dental network. During the first 60 days of your membership, if you are not satisfied with the network for any reason, we will refund your membership fee, minus a $10 service charge to cover a portion of our administrative costs.

| Important Information |
Membership fee preauthorization: Upon enrollment, you authorize New York Dental Network, LLC to charge your credit or debit card initial and renewal membership fees. These pre-authorized and recurring monthly, quarterly or annual payments will continue until further notice from you. We will notify you of future payment transactions via email, and we will remain available by phone and email to help with account changes or service questions.
Immediate Membership Confirmation and Member I.D. documentation will be sent to each new member via printable email. The plan can be used the same day as enrollment, in case of dental emergency, etc. Eligibility will be verified each time services are rendered. If membership cannot be verified, the dentist is not obligated to offer reduced network fees.

| Coordination of Benefits |
In some cases, network benefits can be coordinated with benefits from conventional dental insurance plans. This may result in additional savings. Please discuss this with your dentist in advance of treatment. Here are a few examples, actual rates may vary:
Conventional dental insurance pays 50% for major services up to maximum annual benefit of $1,500. The dental charges are for Orthodontic services (braces) with UCR cost of $5,500. Conventional dental insurance will pay a maximum one-time fee of $1,500 for braces, leaving a balance of $4,000.
If the dental network fee for braces is $4,500, conventional dental insurance pays the same $1,500, leaving a balance of $3,000. The dentist receives his network fee in full, and the member savings is $1,000.
Conventional dental insurance pays 50% for major services up to maximum annual benefit of $1,500. The insured needs root canal and a crown. Normal combined cost for these two procedures is approximately $2,500. Dental insurance will pay 50% or $1,250, leaving a balance of $1,250.
If the combined network charge for these two procedures is approximately $1,800, conventional dental insurance will pay 50% or $900, leaving a balance of $900. The dentist receives his network fee in full, and the member saves $350.
Cost and savings figures given in the above example are approximate, and for illustration purposes only. In cases where coordination of benefits may require additional paperwork or administrative services on the part of the dentist, an administrative fee of $25 to $50 may apply. Please review this with your dentist in advance of services.
Coordination of benefits may extend the utilization of conventional dental insurance, since coordinated benefits are calculated on the basis of lower network fees, per schedules posted on this site.
Since the network has NO maximum limit, network benefits will continue long after conventional dental insurance limits of $1,000 or $1,500 are reached. |
